On October 3, 2005 the Madison Township Fire Department responded to a report of an auto accident (car vs. semi) at 317 & Groveport Road. The Medic, Engine, & Battalion were dispatched from Hamilton Road Station 181 along with Columbus Rescue 4 from Winchester Pike.

Medic 181 arrived first and discovered 2 patients, one still inside the car with a semi tractor trapping them in their seat. They began a rapid assessment of the two patients' condition to determine who needed treatment first based on severity. Since neither patient's condition was immediately life threatening they were able to attend to both of them at the same time. One paramedic dedicated to the treatment of each patient.

Battalion 181 arrived next and requested an additional medic for the second patient and an additional rescue since a prolonged heavy extrication was possible. MedFlight was also put on standby in case the trapped patients' condition worsened or it took a long time to remove them from the car.

Engine 181 arrived and pulled a hose line from their truck for protection against any fire hazards and began extrication attempts with their newly acquired
Holmatro Tool. Initially the patient in the car stated that they were pinned between the door and the dashboard of the vehicle.

Rescue 4 arrived a few moments later and took over extrication efforts with their more advanced heavy duty rescue tools while Engine 181's crew assisted them. MedFlight was put in the air by Battalion 181 after consulting with the Medic crew.

Hamilton Township Medic 172 arrived as the second medic and transported the patient who was already out of the vehicle to Grant Hospital for treatment.

Rescue 2 arrived at the scene, having responded from downtown Columbus, and began assisting Rescue 4 and Engine 181 with the rescue. EMS 14, a medical supervisor from Columbus also arrived and contacted MedFlight to arrange a landing zone and relay the patient's condition to the crew in flight. SO2, a safety officer from Columbus arrived and oversaw the scene for any safety concerns.

After a few more minutes of continued extrication efforts the patient was removed from the vehicle. Crews placed the patient on a backboard and carried them to Medic 181 for evaluation. MedFlight landed on 317 a few moments later and the patient was moved to their helicopter and was transported to OSU Medical Center.
